Colfax Health and Rehabilitation Center is now Colfax Senior Living LLC, which is managed by Frontida Management Group, a family-owned, Wisconsin-based company. Frontida manages 10 other long-term care communities throughout Wisconsin.
“My team and I are excited to continue to serve the community of Colfax and Dunn County and support Executive Director, Amber Parzyck, and her amazing staff,” CEO Crystal J. Miller announced today.
Parzyck, a registered nurse who grew up in the area, has been at the location in various roles since 2021.
“I love that Frontida has helped me continue to do the work that my staff and I love—to provide care for our elders,” Parzyck said.
To assist Parzyck, Frontida hired long-time local resident, Kayla Kunsman to help families during their loved one’s decision-making and move-in experience.
“We are thrilled to have hired Kayla, who knows this community and will be an asset to them and us,” Miller said.
The community is now accepting referrals for assisted living, and long-term care, including memory care, Miller said.

For long-term and memory care, Colfax Senior Living services individual experiencing advanced aging; dementia and Alzheimer’s; physical disabilities and terminal illness. All applications are screened to determine whether the level of care needed corresponds with what Colfax Senior Living offers, Miller added.

Outpatient Therapy Now Offered
“We are also offering outpatient therapy for both Colfax Senior Living residents and the community through our partners at Paxxon Healthcare Services,” Miller said. “This offering will take place in our spacious, fully equipped therapy gym.”
Paxxon provides physical, occupation and speech therapy services customized to each individual’s needs and goals.
